Program Guidelines
What’s included:
Full evaluation, imaging and diagnosis services at our locations.
Surgical procedure (if required) with facility, physician, anesthesia and implant costs covered by the bundle.
Follow-up care within the defined post-treatment time period (for example, 90 days).
What’s not included:
Diagnostic tests before or after the procedure that aren’t part of the defined bundle (lab work, extra imaging).
Therapy beyond standard follow-up if extended or non-routine care is needed.
Bracing, durable medical equipment, or treatment of complications outside the bundled timeframe.
Care related to workers’ compensation claims or conditions outside the typical service bundle.
