Payment
Bills related to general anesthesia will be issued by Anesthesia Services. This is the fee from your Anesthesiologist.
Finding Your Guarantor Number
When making a payment or setting up your account, you’ll need your Guarantor Number. Look at the upper right-hand corner of your billing statement, near the credit card information section. Your Guarantor Number will begin with a “1” or a “2.” If you have trouble locating your Guarantor Number, please contact our billing department for assistance.
New Patient/Medical Insurance Plans
New patients are welcome at Orthopaedic Associates of Wisconsin. We accept most major health insurance carriers, Government plans, Medicare Part B, Wisconsin Medicaid (specific plans), self-pay, HealthShare members, and workers' compensation plans. Please contact your insurance carrier for more information or call us at 262-303-5055 ext 3004. When contacting your insurance, Orthopaedic Associates of Wisconsin's Tax ID is 391-101-566.
Your Rights Against Surprise Medical Bills
What You Need to Know
You’re protected from surprise or balance billing when receiving emergency care or certain services at an in-network hospital or ambulatory surgical center. Balance billing happens when out-of-network providers charge you the difference between what your insurer pays and what they charge. Under federal and state law, you cannot be billed more than your in-network costs for emergency services or care from certain specialists—even if they're out-of-network. Request a Good Faith Estimate for Medical Costs
If you’re uninsured or not using insurance, you have the right to receive a Good Faith Estimate that outlines the expected cost of your medical care. This estimate includes all related expenses for non-emergency services, such as tests and equipment. You can also request one in advance.
